MOSCOW, August 16 (Itar-Tass) — The August 6 abortive launching of the Proton-M carrier rocket was connected with malfunctioning of the Breeze-M accelerator block, which, in turn, was a result of a production fault, Chief of the Federal Space Agency (Roscosmos) Vladimir Popovkin told journalists on Thursday.

An interdepartmental commission which is investigating the causes of the failure has been working since August 7, Popovkin said. "I dare say that the causes of the accident were established within ten days. It was connected with a production fault," Roscosmos chief said. He explained that the Breeze-M had a pressurization system of extra fuel tanks blocked.

The Proton-M carrier rocket provided with the Breeze-M accelerator block, which was launched from Baikonur cosmodrome at 11. 31 pm Moscow time on August 6, was to have brought two space apparatuses to the assigned orbit. One of them was the Russian "Express Md-2 "apparatus and the other one was the Indonesian satellite Telkom-2; both apparatuses failed to reach the assigned orbit.
http://www.itar-tass.com/en/c154/497572.html

Russian Prime Minister Dmitry Medvedev chaired a meeting of the problematic issues of organization of space activities
08/14/2012 ::

Opening Remarks Medvedev:

Dmitry Medvedev: The subject is well known. We have, unfortunately, an accident during launch spacecraft come with an enviable degree of regularity. Over the past year and a half there were seven launches emergency and had lost 10 satellites. If this is compared with the results of other leading space powers, then, unfortunately, the difference is huge, even with the understanding that these launches - it is always a certain set of challenges and risks. However, nowhere anything like this happens. The most recent example: August 6, the case of carrier rocket "Proton-M". Wasted as a result of all this, billions of rubles. And what is particularly unpleasant, of course, is that there is a weakening of authority in our country as a very large, the leading space power. And I want to note that this situation is, despite the fact that every year we are investing in the development of space industry becoming more and more heavily. I recall that from 2012 to 2015 is planned to invest 650 billion rubles. We need to understand what is happening, what the causes of those who responded to everything that has happened, except for the unfortunate of the insurance company. I then figured out, it turns out that after the payment of insurance compensation insurance premium payments to the consequences, even in no way are redistributed to the real culprits. It is possible to produce low-quality products, it is an accident, to put it gently, and then the enterprises themselves, which to some extent to blame for the incident, no financial responsibility does not carry, not to mention other responsibilities, I mean disciplinary and other. You need to decide who is to blame for a series of recent setbacks, where mistakes were made, and in any case to determine the degree of responsibility of all implicated in these problems. Part of the decisions I take today.

Russia has traditionally had a very significant potential in space exploration. Created by our production capacity is our competitive advantage, despite the difficulties. Only three countries now - Russia, the United States and China - have all the technologies and capabilities for manned flights. Of course, we all know that the industry, there are systemic problems that these problems are not just subjective. First - this is an old industrial base: 90% of capacity in operation for over 20 years, they must be replaced. Second - this is a weak base is an electronic component, which is a common disaster for our industry, and for the space industry in particular. In addition, our rocket and space technology is not fully secured the necessary construction materials, and it is also a problem. All this leads to various kinds of failures at various stages of operation. And the third problem - Personnel: lack of skilled workers, the aging of scientific and operational skills. Number of employees up to 30 years in the industry now stands at 20 percent with a little - it's a real problem.

However, what I said does not justify the developments in recent years, failure to launch satellites. Vladimir (V.A.Popovkin, Head of Federal Space Agency), you are going to report on the situation, including the practical steps that are envisaged to improve the quality of the industry, including (would like to draw your attention) and Questions about the content and the organizational form of the Russian Space Agency. Issues of quality control of products also remain an acute problem. Both of these issues must be worked out at the government level in a month, and then I will hold a meeting with all key business sectors. Other decisions will be taken. Let's start the discussion. Thank you all.

During the emergency commission on August 6 launch of abnormal carrier rocket "Proton-M" in her comments have appeared not only to the upper stage, but also to install the third stage motor vehicle, issued several recommendations to improve the reliability of this unit, told journalists on Thursday Head of Russian Federal Space Agency Vladimir Popovkin.
"I have to say one more thing. I have instructed the Commission to consider an emergency full-flow of telemetry from the launch site prior to an emergency event. In this analysis, there were some comments to the third stage propulsion system," - said Popovkin.
"Although they did not lead to abnormal operation of the engine, however, I have instructed more carefully it all worked out. Issued a number of recommendations to improve the reliability of the propulsion system of the third stage", - concluded the head of the Russian Space Agency, RIA Novosti reported.

19.08.2012 / 00:05 Roscosmos should go to the European Quality Management - Expert

Russian space industry must move to a European multilevel system of quality management, as well as to adopt a system of staff motivation - but it will reduce the risk of accidents, ex-head of the Russian Aerospace Agency, Chairman of the Scientific and Technical Council of "Russian Technologies" Koptev, RIA Novosti reported.
Speaking on the occurrence of the recent accidents in space launches, he noted that in the Soviet era, and the new Russian history such accidents occur.
Recently, Russia has had several failed launches of space vehicles, because of what has been lost several satellites and interplanetary probes. The last time this happened on August 7, when abnormally due to failure of the block "Breeze-M" over the launch rocket "Proton-M", to put into orbit communication satellites "Express-MD2" and "Telkom-3."
"But this is by no means justified the situation that has arisen now. And those eighteen months, during which we had seven troubles, they certainly fall out of the statistics and make think about certain system events that can help, and I convinced that allow to preserve the good name of the Russian space industry and Russian, which makes these products "- Koptev said, speaking on the radio station" Echo of Moscow ".
According to him, in the space industry to change the system of quality management. The old Soviet system of military office was destroyed, said Koptev and "intra" control system was not able to keep the quality up to par.
"We have to go to the European system of quality management. Is this experience, we have a well-known organization, located in Siberia ... ISS Reshetnev name. Her life has forced the system to recreate the west, phase control, where at each stage of a process of verification of the results that Incepted in the project. Such steps usually 12-13 stages, allowing early identify deviations "- said Koptev.
In addition, he said, it is necessary to consider and adopt a "system of motivation of the staff working in responsible operations." Koptev recalled that in Soviet times, people who have worked in the system of the rocket and space industry, except for understanding the importance, significance, except for administrative pressure, the party had a financial motivation. Their wages were 15-20% higher than in other industries. Was a powerful social package.
Now wages in the space industry can not expect to attract new staff.
Koptev gave the example of the Khrunichev Center, which has earned for launching foreign satellites $ 770 million, while the average salary in the company 28 thousand rubles, with an average salary in Moscow more than 40 thousand.
"With this salary talk about some motivation .... very difficult to expect that the graduates of leading universities will go to these businesses," - he said.
Koptev considers it necessary to reconstruct the Soviet period, the power vertical in the space industry.
"The first thing I had to do, of course - it's hard to keep the vertical control. Situation today, when the decision to display all federal state enterprises into joint stock companies, the Federal Space Agency, a federal agency, every such decision loses control," - said the expert.
According to him, a possible option would be to use the experience of Rosatom - that is, becoming a state corporation.

22.08.2012 / 00:05 Head of the Khrunichev center remains chief designer

The head of the Khrunichev Space Center Vladimir Nesterov, who resigned because of the accident booster "Briz-M" will be left in the center for the post of the designer. On it informs "Interfax" referring to its sources in the industry.
According to the source, "the solution is particularly appropriate in view of the fact that the finish line out a large-scale project to create a missile" Angara ", for whose realization Nesterov, as head of the Khrunichev center, did a lot."

Rogozin: The former head of the Center. Khrunichev will work on a missile "Angara"
August 27, 2012

Quote:
Former director general of the Khrunichev Space Center, Vladimir Nesterov, who left the post after a failed space launches, will remain in the industry and will develop new machines, said Monday Deputy Prime Minister Dmitry Rogozin.

"I think that in view of the work that we have deployed on a set of" Angara ", Nesterov, probably will remain in the system, the development of new space vehicles," - he said, "Interfax"

08.28.2012 / 00:05 Khrunichev Center screens all boosters "Briz-M"

Khrunichev Center screens all boosters "Briz-M" to prevent a repetition of the accident that led to the loss of communication satellite "Express-MD2" and "Telkom-3", told RIA Novosti the Centre.
"The emergency commission investigating the causes of abnormal situations, requested to check all the boosters" Briz-M "and components for their production," - said the source.

08.28.2012 / 00:05 RKK "Energy" is ready to offer boosters instead of "Breeze"

RSC "Energia" is ready to offer the DM own design instead used on the carrier rocket "Proton" boosters "Briz-M", because of a failure in one of them into an unplanned orbit withdrew Russian and Indonesian communications satellite, told reporters the head RSC "Energia" Vitaly Lopota on Monday on the sidelines of the International Aerospace Congress in Moscow.
"At any time we are ready to provide the DM" - said Lopota.
He stressed that the D-stage is exceptionally high precision breeding.
"Blocks DM flight observations never had after the flight test," - said Lopota, RIA Novosti reported.

Absolument aucune chance.
